To become a registered user, you must receive prescription benefits from a company who has hired PrismRx to provide you with online prescription services. Your employer or PrismRx will provide you with the information you need in order to complete the registration process. There are several areas at Prismpbm.com that are open to the general public; however these areas of the site may also provide additional information or services for registered users.
PrismRx collects personally identifiable information at Prismpbm.com from web visitors who are interested in receiving services it offers, such as prescription benefit, disease management and specialty pharmacy services, e-mail alerts, newsletters, and event notifications. PrismRx also collects additional information from registered users, such as shipping address, billing address, credit card number and other information necessary to process your request. In order to provide you with online pharmacy services, PrismRx receives certain personally identifiable information about registered users from the user’s employer or health plan. PrismRx provides you with access to Prismpbm.com for products and services associated with PrismRx’s pharmacy benefit management services. In order to provide this information to you and for you to access Prismpbm.com, enhance your overall service experience, and for other reasons set forth below, PrismRx, in certain cases, may share with certain of its affiliates all or portions of the information gathered from you through your use of Primpbm.com as described below.

PrismRx works with third-party providers to provide specific services offered on Prismpbm.com, and may collect personal information as part of the provision of those services.

In addition to our official websites, PrismRx works with a number of third-party partners that host websites and micro-sites that provide information and services to our customers. We will disclose your personal information if, in good faith, we believe we are required by law to do so, including but not limited to disclosure to law enforcement or other government officials in connection with an investigation of fraud, intellectual property infringement, or other activity that is illegal or may expose us to legal liability.
